Denise Hughes is the Program Manager, School Initiatives in the Nemours Value Based Services Organization. In this role, Ms. Hughes develops, implements, and evaluates programmatic initiatives that have a direct impact on the populations Nemours serves. She accomplishes this by using health outcomes data and determining which areas require the greatest attention, especially as Nemours begins to take on risk. Throughout her 21 years with Nemours, Ms. Hughes has leaded numerous Population Health initiatives and research projects with schools. She has worked with other Nemours associates as well as community organizations to help Nemours in achieving their mission.
Her work with schools has a broad reach. She is leading the opening of 9 Elementary School-Based Health Centers and is assisting Nemours achieve their whole child strategy and reaching patients and families where they are. She works closely with School District personnel to establish connections between schools and the health care system. She is leading an initiative to ensure students attendance data is shared via a health information exchange with their primary care provider (PCP) to determine the underlying/root cause of the absence. Denise also works with school nurses across the Delaware Valley to enable them access to student medical records, with parent permission, to allow them to be an extension of the PCP.
