Rev. Alexander Plum, MPH, CHES oversees Henry Ford Health System’s efforts to address the social determinants of health across all facets of care delivery. As Director, Clinical and Social Health Integration, he establishes and leads programs designed to connect integrated care teams with community social services to sustainably mitigate social barriers to health, with special attention to operational sustainability and value-based contracts. A Deacon in the United Methodist Church, Rev. Plum builds partnerships in Michigan and around the world to equip local churches to promote health in their communities. In his five years at Henry Ford, he has established himself as a nationally-recognized expert in the field of ‘reverse innovation’ in healthcare, securing and managing $4M in external funding to test innovative healthcare delivery models that originated in low-income settings. Alex is a Salzburg Global Fellow, a former Coverdell Fellow, and a returned Peace Corps Volunteer (Micronesia, 2008-2011). He is a 2015 alumnus of the Rollins School of Public Health at Emory University (MPH, Behavioral Science & Health Education), which honored him that year with the University Humanitarian Award. Alex graduated Phi Beta Kappa from Michigan State University (BA, International Relations & Political Economy) in 2008.
