Hilda Perea

Hilda Perea, Vice President of Care Management for Independent Living Systems, currently oversees the care management of Dual Special Needs members. She has led the NCQA accreditation of ILS’ current Health Risk Assessment and Long-Term Support Services Care Management programs, and supports corporate and state audits, as well as other ILS departments.

Hilda previously served as the Director of Care Management, overseeing several CMS and commercial contracts. She has also assisted IT with enhancements to eCare, the award-winning Care Management Platform, improving and updating the web-based application to meet different state requirements.

A Registered Nurse by training, she worked at Mercy Hospital for 13 years as a telemetry nurse, patient educator. During the tenure there, she established in-house post-discharge Cardiac Rehabilitation and Patient education programs in conjunction with cardiologists and cardio thoracic surgeons.
