Gabriela Segovia MHA

Gabriela Segovia is the Director of Ambulatory Care Management at EPIC Management L.P, serving Los Angeles County, San Bernardino County, and Riverside County, in Southern California. She received her Bachelors of Science in Organizational Leadership & Management, and Master’s in Health Administration from the University of La Verne. Gabriela has a diverse work history, ranging from managed care, academic medicine, medical group management and higher education. In her role at EPIC Management, Gabriela leads the Population Health Program, Social Services Program, Transition of Care Program, Care Management Program, Complex Care Management Program, Transitional Care Management Program, and Provider to Home Program. Her passion is improving patient outcomes by assessing their clinical health, health behaviors, and social determinants of health.
